The Verizon Digital Wellness FIFA World Cup 2026™ Tickets Sweepstakes (“Sweepstakes”) starts on April 13, 2026, at 12:00 a.m. Eastern Time (ET) and ends at 11:59 p.m. Eastern Time (ET) on the dates listed below for each designated Sweepstakes location (the “Sweepstakes Period”). By participating in the Sweepstakes, each entrant unconditionally accepts and agrees to comply with and be bound by these Official Rules and the decisions of Verizon Corporate Services Group Inc. (“Sponsor”), which are binding and final in all respects.

1. ELIGIBILITY

The Sweepstakes is open to individuals who: (i) participate in a qualifying Verizon Digital Wellness Event [i.e., Older Adults Workshop, Family Day, or Community Day]; (ii) are at least 18 years of age at the time of entry in the Sweepstakes; (iii) are citizens or legal residents of the United States; and (iv) are NOT in one or more of the “Prohibited Persons” categories listed below. Any entry submitted by any ineligible person(s) will be void.

Participant Verification:

• Must register electronically for a qualifying Verizon Digital Wellness Event in a select city (listed below) using the online Verizon Digital Wellness events registration microsite. During registration, must select YES to the question asking if you would like to enter the Verizon Digital Wellness FIFA World Cup 2026™ Tickets Sweepstakes.

• Must attend the Verizon Digital Wellness Event in that city and meet the following criteria based on event type:
  
  • Older Adult Workshop: Take part in a Verizon Digital Wellness Older Adult Workshop offered at a Verizon retail store participating in the Sweepstakes – verification includes on-site check in and attendance during a presentation by the workshop facilitator.
  • Family Day: Take part in a Verizon Digital Wellness Family Day offered at a Verizon retail store participating in the Sweepstakes – verification includes on-site check in and engagement at four facilitator-led activity stations while collecting stamps on their Digital Wellness Activity Passport.
  • Community Day: Attend a Verizon Digital Wellness Community Day hosted in a participating Sweepstakes city – verification includes on-site check in and engagement at two facilitator-led activities while collecting stamps on their Digital Wellness Prescription Pad.
• Must agree to signing an affidavit verifying identity (photo ID required) and completion of eligibility requirements for Sweepstakes, if selected as a winner.

Prohibited Persons. The following individuals are not eligible to enter or win the Sweepstakes: (i) current and former employees (including, but not limited to, consultants, independent contractors, temporary employees, and interns) of Sponsor, onsite Sweepstakes administrator, (“Administrator”), and of their respective parents, subsidiaries, affiliates, authorized agents, retailers, sales representatives, distributors, suppliers, advertising agencies, promotion agencies (hereafter collectively, “Sweepstakes Entities”) and immediate family members of each such individual; and (ii) all individuals (including, but not limited to, consultants, independent contractors, temporary employees, former employees, and interns) who have, within the past six months, performed services for Sweepstakes Entities. “Immediate family” members include spouses, grandparents, parents, children and grandchildren, whether by marriage, past marriages, remarriage, adoption, cohabitation or other family extension.

2. HOW TO ENTER

During the Sweepstakes Period there are three ways to enter:

• In advance, go directly to the online Verizon Digital Wellness events registration microsite located at digitalwellness.verizon.com and accurately complete a registration for an eligible event.
• In advance, use an internet-enabled mobile device to scan the QR code on Sweepstakes marketing materials to connect to the Verizon Digital Wellness events registration microsite and accurately complete a registration for an eligible event.
• While on-site at an eligible Verizon Digital Wellness Event, go to the event registration microsite and accurately complete an online registration. On-site walk-in registrations will only be accepted if a space is available.

Registrants who are no-shows for their digital wellness event will no longer be entered in the Sweepstakes.

In each of the three entry options, once you have completed the foregoing, you must select AGREE to the Official Rules and Privacy Policy for the Sweepstakes. You must also select YES to the question asking if you would like to enter the Verizon Digital Wellness FIFA World Cup 2026™ Tickets Sweepstakes. Sweepstakes entry must be part of a registration for and participation in an eligible Verizon Digital Wellness Event. Once online registration for an eligible event has reached capacity, if applicable, the opportunity to enter the Sweepstakes via the Verizon Digital Wellness events registration microsite will be closed. Data usage rates may apply to access the Verizon Digital Wellness events registration microsite using a mobile device.

3. CONDITIONS APPLICABLE TO ALL ENTRIES

Limit one (1) entry per person and per email address. Sponsor’s computer is the official time-keeping device for the Sweepstakes. All received entries become the exclusive property of Sponsor and will not be acknowledged or returned, and no correspondence will be entered into. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and/or logins, or any other methods will void that entrant’s entries and that entrant may be disqualified if discovered by Sponsor. The use of automated entry devices or any other conduct that impedes the integrity of the Sweepstakes is prohibited. Any individual who attempts to enter using false contact information or who uses any unauthorized device or artifice to enter using a false identity, as determined by Sponsor, will be disqualified. If there is a dispute as to who submitted an entry, the entry will be deemed submitted by the Authorized Account Holder of the email address submitted at the time of entry. “Authorized Account Holder” is defined as the natural person assigned to an email address by an internet access provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the email address in question.

Sweepstakes Entities are not responsible for late, lost, incomplete, misdirected, stolen, or damaged entries or for technical, hardware, software or device malfunctions of any kind, lost or unavailable network connections, or failed, incorrect, incomplete, inaccurate, garbled or delayed electronic communications caused by the sender or by any of the equipment or programming associated with or utilized in the Sweepstakes that may limit the ability to participate, or by any human error that may occur in the processing of the entries in the Sweepstakes or other failures of any sort, fire, epidemic or pandemic, explosion, act of God, riot or civil disturbance, war, labor strike, terrorist threat or activity, any federal, state or local government law, or any other cause not reasonably within Sweepstakes Entities’ control.

4. WINNER SELECTION

Random Drawing. Within approximately one week from the completion date of the applicable Verizon Digital Wellness Event, potential prize winners will be selected from the combined pool of entrants for that city via random drawing by a representative from the organizations facilitating the Verizon Digital Wellness events. The selected winner(s) will be chosen at the sole discretion of the Sponsor, from among all eligible entries received during the allotted time period for the Sweepstakes and in adherence with the aforementioned criteria. Potential prize winners must meet all eligibility requirements and comply with these Official Rules to qualify for the prize. The judging organization’s decisions are final on all matters relating to the drawing. Limit one prize (2 Tickets) per person during the Sweepstakes Period.

5. PRIZES / ODDS OF WINNING

Prize. Each verified Winner will receive two (2) tickets to the FIFA World Cup 2026™ based on the city in which they entered.

• Two (2) Winners from Miami will receive tickets to Match 86 on Friday, July 3, 2026
• Three (3) winners from Boston will receive tickets to Match 97 on Thursday, July 9, 2026
• One (1) winner from Los Angeles will receive tickets to Match 98 on Friday, July 10, 2026
• Three (3) Winners from New York City will receive tickets to Match 104 (World Cup Final) on Sunday, July 19, 2026

Combined ARV of all Prizes is $66,050.

• Friday, July 3 (Match 86 - Miami) - $1,400 ($700 per Ticket)
• Thursday, July 9 (Match 97 - Boston) - $2,540 ($1,270 per Ticket)
• Friday, July 10 (Match 98 - Los Angeles) - $3,550 ($1,775 per Ticket)
• Sunday, July 19 (Match 104 - World Cup Final, New York City) - $17,360 ($8,680 per Ticket)

Parking, travel, and accommodations are not included in the Prize and are the sole responsibility of the winner and guest. Alcoholic beverages are not part of the Prize. All expenses and costs not expressly listed above, including, but not limited to, federal, state or local taxes on the value of the prize, are the sole responsibility of the winner. Prize is not transferable, assignable, or redeemable for cash except at Sponsor’s sole discretion. Prize details and availability are subject to change, in which case Sponsor may substitute a prize of equal or greater value. Any portion of prize not used by the winner is forfeited and no cash substitute will be offered or permitted. Prizes are subject to all additional terms and conditions thereon. Prize offered is provided “as is” with no warranty or guarantee either expressed or implied by Sponsor. Sponsor is not responsible for loss, delay, or damage in delivery of Prize. All properly claimed prizes will be awarded provided a sufficient number of eligible Entries are received but in no event will more prizes be awarded than are provided for in these Official Rules.

Odds of Winning. The odds of winning depend upon the number of eligible entries received during the Sweepstakes Period.

6. FIFA TICKET TERMS

Winners and/or their guests of any Game (“Attendees”) must be eighteen (18) years or older or accompanied by an adult. Attendees agree to comply with the FIFA Ticket Terms of Use (“Terms of Use”) applicable to their ticket(s) to the FIFA World Cup 2026™ (“FWC26”), including any restrictions on the ticket holder’s ability to resell and transfer such tickets that may apply. The Terms of Use and any other ticket terms and conditions are available (or will be made available) on FIFA.com/tickets. Please note that the applicable version of the Terms of Use may vary depending on the location of the relevant FWC26 match to which a ticket corresponds. Ticket holders are encouraged to check for any updates to the Terms of Use, and any other ticket terms and conditions regularly. In accordance with the Terms of Use, FIFA reserves the right to cancel any ticket(s) if FIFA determines that an Attendee has failed to comply with the terms and conditions applicable to tickets. Attendees must also comply with the FWC26 Stadium Code of Conduct (“Stadium Code of Conduct”) (also available on fifa.com/tickets) and all other venue health and safety protocols and are expected to respect other guests and staff at the event. You, as Winner and your guest(s) are expected to respect other guests and staff at the event and refrain from using foul or offensive language, obscene gestures, or behavior that is unruly, disruptive, threatening or violent in nature. In this respect, the rules governing FWC26 stadium safety, security and conduct, set out in the Stadium Code of Conduct, the Terms of Use and all other applicable terms and conditions available on fifa.com/tickets are deemed to be incorporated into these Terms and Conditions. FIFA reserves the right to cancel any ticket to the event, deny entry to, or remove, any Attendee from the venue if Attendee engages in such behavior. Further, Verizon and/or FIFA reserve the right to prohibit an Attendee from participating in future Verizon-sponsored FIFA experiences or attending future editions of FIFA competitions (as applicable). By using a ticket to attend a match, each Attendee consents to their person and belongings being searched prior to and/or after entry into the Stadium.

7. WINNER NOTIFICATION PROCESS

Sponsor will attempt to notify potential winners via email and/or phone, using information provided on the Entry form on the Verizon Digital Wellness events registration microsite and gathered on-site at an event via an affidavit. Potential winners will be required to follow prize claim instructions provided and return all necessary prize claim documents within five (5) days from the date of notification. Potential winners must complete and sign a Prize Acceptance & Liability Waiver and a Publicity Release before the Prize will be awarded (TN residents need not sign the Publicity Release). The Prize will be forfeited in the event that: (i) the potential winner is not in compliance with these Official Rules, including any eligibility requirements; (ii) the prize notification or attempted notification is returned as undeliverable; (iii) Sponsor does not receive timely response to prize notification; or (iv) a potential winner declines the Prize offered. If time permits, Sponsor may select an alternate potential winner by applying the criteria set forth above.

Each Winner will be issued an IRS 1099 Tax Form reflecting the fair market value of their Prize; and, Winner agrees to fully cooperate with Sponsor relative to its reporting of prize award to the Internal Revenue Service, including provision of his/her Social Security Number in conjunction therewith.

8. GENERAL RULES

By accepting the prize, the winner grants to the Sweepstakes Entities the right to use winner’s name, likeness, image, voice, hometown and biographical information for purposes of advertising, trade and promotional content in any and all media now known or otherwise developed, including on the internet or via wireless application protocol, without further notice, compensation, permission, or review, including a winner’s list, except where prohibited by law. (TN residents need not consent.) Government employees who win a prize may be subject to ethics and gift rules of their jurisdiction. Sweepstakes Entities are not responsible for printing errors in these Official Rules. In the event there is a discrepancy or inconsistency between disclosures or other statements (in any and all media/medium) contained in any Sweepstakes materials and terms and conditions of the Official Rules, the Official Rules will prevail, govern and control.

Sponsor reserves the right to disqualify any individual from further participation in the Sweepstakes if Sponsor concludes, in its sole discretion, that such person: (i) has attempted to tamper with the entry process or other operation of the Sweepstakes; (ii) has disregarded or has attempted to circumvent these Official Rules; (iii) has committed fraud or attempted to undermine the legitimate operation of the Sweepstakes; or (iv) has acted toward any Sweepstakes Entity or any other entrant in an unfair, inequitable, annoying, threatening, disrupting or harassing manner. Tampering includes attempting to enter more than the number of times permitted herein, including through the use of any prohibited device or method. If a dispute arises regarding compliance with these Official Rules, Sponsor may consider, in its sole discretion, data reasonably available to Sponsor through information technology systems in Sponsor’s control, but Sponsor will not be obligated to consider any data or other information collected from any other source. Any attempt by any person to deliberately undermine the legitimate operation of the Sweepstakes may be a violation of criminal and civil law, and should such an attempt be made, the Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. The Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that or any other provision.

If for any reason, the Sweepstakes is not capable of running as planned, including, but not limited to, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes, then the Sponsor reserves the right in its sole discretion to cancel, terminate, modify, or suspend the Sweepstakes. If Sponsor cancels, terminates, modifies, or suspends this Sweepstakes for any reason, Sponsor may select potential prize winners by random drawing from among all eligible, non-suspect entries received during the Sweepstakes Period prior to termination, or as otherwise deemed fair and appropriate by Sponsor in its sole discretion. If Sponsor does so, Sponsor will post notice of its action at digitalwellness.verizon.com/.

9. INDEMNIFICATION / LIMITATIONS OF LIABILITY

By participating in the Sweepstakes, entrants agree to release, waive and hold harmless Sweepstakes Entities, and each of their respective affiliates, subsidiaries, parent corporations, retailers, sales representatives, distributors, and advertising and promotional agencies, and all of their officers, directors, shareholders, employees and agents, from all injuries, claims, damages, losses, costs or expenses of any kind (including, without limitation, attorneys’ fees) resulting from the entrants’ participation in this Sweepstakes, the entrants’ acceptance, use or misuse of any prize or element thereof, (including any travel related thereto), including installation of the prize elements, and any travel or event-related activity in connection with the Sweepstakes. Neither the failure of Sponsor to insist upon or enforce strict performance of any provision of these Official Rules or the failure, delay or omission by Sponsor in exercising any right with respect to any term of these Official Rules, will be construed as a waiver or relinquishment to any extent of Sponsor’s right to assert or rely upon any such provision or right in that or any other instance.

10. GOVERNING LAW

ALL ISSUES AND QUESTIONS CONCERNING THE CONSTRUCTION, VALIDITY, INTERPRETATION AND ENFORCEABILITY OF THESE OFFICIAL RULES OR THE RIGHTS AND OBLIGATIONS OF ENTRANTS OR SPONSOR IN CONNECTION WITH THE SWEEPSTAKES WILL BE GOVERNED BY AND CONSTRUED IN ACCORDANCE WITH THE INTERNAL LAWS OF THE STATE OF NEW JERSEY, USA WITHOUT GIVING EFFECT TO ANY CHOICE OF LAW OR CONFLICT OF LAW RULES OR PROVISIONS THAT WOULD CAUSE THE APPLICATION OF ANY OTHER STATE’S LAWS.

11. DISPUTE RESOLUTION

By entering the Sweepstakes, entrants agree that: (i) all disputes, claims, and causes of action arising out of or connected with the Sweepstakes, or prizes awarded, will be resolved individually, without resort to any form of class action, and exclusively by the appropriate court in New Jersey, to which the participant submits to personal jurisdiction; (ii) all claims, judgments, and awards will be limited to actual out-of-pocket costs incurred, including costs associated with entering the Sweepstakes but in no event attorneys’ fees; and (iii) under no circumstances will any entrant be permitted to obtain any award for, and entrant hereby waives all rights to claim, punitive, incidental or consequential damages, and all rights to have damages multiplied or otherwise increased, and any other damages except for actual out-of-pocket expenses. Some jurisdictions do not allow for limitations on the ability to pursue class action remedies, or certain kinds of damages, and so these limitations may not apply to you.

12. PERSONAL INFORMATION

Any personal information supplied by you to the Sweepstakes Entities will be subject to Sponsor’s privacy policy posted at verizon.com/about/privacy/full-privacy-policy. By participating in the Sweepstakes, you authorize Sponsor to communicate with you by text message using your wireless phone number provided in connection with the Sweepstakes.

13. WINNERS LIST

You may request the name of the winner(s) by sending an email with the name of the Sweepstakes and Winners List in the Subject Line to: winnerslist@probabilityworks.com, or a self-addressed, stamped envelope with Verizon Digital Wellness FIFA World Cup 2026™ Tickets Sweepstakes Winners List, c/o Probability, PO Box 1559, Social Circle, GA 30025-1559. Requests must be received no later than 30 days after the Sweepstakes ends.
